Plan Types by Carrier
Most large carriers offer both HMO and PPO (or EPO) products. Kaiser is HMO-only. Sharp and Sutter Health Plus are HMO-only regional plans. Oscar offers EPO/HMO products. Cigna's flagship is Open Access Plus EPO/PPO. Anthem has the broadest PPO network. Blue Shield's Access+ HMO is distinctive for HMO-without-medical-group-restriction flexibility.
