EPO Plans: The HMO-PPO Hybrid

An EPO (Exclusive Provider Organization) combines features of both HMO and PPO plans. Like a PPO, EPOs do not require a Primary Care Physician designation or specialist referrals — employees can self-refer to any specialist in the network. Like an HMO, EPOs provide no coverage for out-of-network providers except in genuine emergencies.

Cigna Open Access Plus EPO has a strong California presence with good provider networks in urban markets. UnitedHealthcare offers Choice EPO products in CA. Network Adequacy and Emergency Coverage

ACA requires all EPO plans to cover emergency services at in-network cost-sharing levels regardless of whether the ER or treating physician is in network. This "no balance billing for emergencies" rule protects EPO members from the worst out-of-network scenario. California's network adequacy standards also require EPO plans to maintain sufficient providers within reasonable geographic distances.

EPO vs PPO: Is the Savings Worth It?

For employees who rarely use out-of-network providers, an EPO provides essentially the same experience as a PPO at lower cost. The risk is an unexpected out-of-network situation — a procedure at an in-network hospital with an out-of-network surgeon, for example. California's surprise billing protections (implementing federal No Surprises Act) limit some of this risk.
